不同体质指数早发2型糖尿病的临床特征分析

Clinical characteristics of early-onset type 2 diabetes mellitus patients with different body mass index level

  • 摘要: 目的:比较不同体质指数(body mass index, BMI)早发2型糖尿病(T2DM)的临床特征。方法:选择临床初次诊断为T2DM的年龄≤35岁的患者287例,按BMI不同分为超重组(BMI≥24 kg/m2)190例和非超重组(BMI<24 kg/m2)97例,比较两组患者在一般资料和生化指标方面的差异。结果:超重组患者以男性为主,高血压、非酒精性脂肪性肝病(non-alcoholic fatty liver disease, NAFLD)的发生率均明显高于非超重组(P<0.01)。超重组患者的空腹血糖(FPG)、空腹胰岛素(FINS)、餐后2 h胰岛素(2 h INS)、空腹C肽(FC-P)、餐后2 h C肽(2 h C-P)、胰岛素抵抗指数(HOMA-IR)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、γ-谷氨酸转肽酶(γ-GT)、总胆固醇(TC)、三酰甘油(TG)、血肌酐(SCr)、血尿酸(UA)水平均高于非超重组(P<0.01);超重组患者的糖化血红蛋白(HbA1c)、糖化白蛋白(GA)、高密度脂蛋白胆固醇(HDL-C)水平较非超重组低(P<0.05)。结论:超重的早发T2DM患者发生高血压、NAFLD的比例高,肝肾功能减退、胰岛素抵抗和代谢功能紊乱较严重。

     

    Abstract: Objective:To compare the clinical characteristics of early-onset type 2 diabetes mellitus (T2DM) patients with different body mass index (BMI). Methods:Totally 287 cases of initial-diagnosed T2DM patients (age≤35 years old) were divided into overweight group (BMI≥24 kg/m2, n=190) and normal-weight group (BMI<24 kg/m2, n=97). The general information and biochemical indexes were compared between the two groups. Results:Compared with the normal-weight group, the overweight group had significantly higher proportion of male patients, significantly higher prevalence of hypertension and non-alcoholic fatty liver disease (NAFLD, P<0.01). Compared with normal-weight group, the patients in overweight group had significantly higher levels of fasting glucose (FPG), fasting serum insulin (FINS), postprandial 2-hour insulin (2 h INS), fasting C-peptide (FC-P), postprandial 2-hour C-peptide (2 h C-P), insulin resistance index (HOMA-IR), alanine aminotransferase (ALT), aspartate aminotransferase (AST), γ-glutamyl transpeptidase (γ-GT), total cholesterol (TC), triglyceride (TG), serum creatinine (SCr), and uric acid (UA, P<0.01). While the levels of glycated hemoglobin (HbA1c), glycated albumin (GA), high density lipoprotein cholesterol (HDL-C) were significantly lower than those in the normal-weight group (P<0.05). Conclusions:Early-onset and overweight T2DM patients have a higher prevalence of hypertension and NAFLD, with more serious liver and renal dysfunction, HOMA-IR, and metabolism disorders.
