松果体区域占位合并神经精神症状的研究进展

Research progress on pineal region space-occupying lesions complicated by neuropsychiatric symptoms

  • 摘要: 松果体区域占位性病变(pineal region space-occupying lesion, PR-SOL)是一组罕见的颅内异质性病变,临床表现不仅包括因肿瘤压迫中脑导水管所致梗阻性脑积水引发的典型神经系统症状,还常伴随一系列认知障碍、情绪与行为异常,以及头痛、睡眠障碍等复杂的神经精神症状。这些神经精神症状表现多样且缺乏特异性,易掩盖颅内病变,导致诊治延迟,是影响患者功能恢复与长期预后的重要临床问题。本文阐述PR-SOL患者神经精神症状的表现形式,以及其涉及的颅内压增高、神经结构受压、内分泌紊乱及神经炎症的多重病理机制,并探讨以病因治疗为基础、结合多学科协作的综合管理策略,以期为PR-SOL及相关神经精神症状临床识别与干预提供参考。

     

    Abstract: Pineal region space-occupying lesions (PR-SOL) are a group of rare and heterogeneous intracranial lesions. Their clinical manifestations include not only typical neurological symptoms resulting from obstructive hydrocephalus caused by tumor compression of the cerebral aqueduct, but also a range of complex neuropsychiatric symptoms such as cognitive impairment, emotional, behavioral disturbances, headache and sleep disorders. These neuropsychiatric symptoms are diverse and nonspecific, often masking the underlying intracranial pathology and leading to delays in diagnosis and treatment. They represent a significant clinical issue affecting patients’ functional recovery and long-term prognosis. This review describes the clinical presentations of these neuropsychiatric symptoms, and the multiple pathological mechanisms involving increased intracranial pressure, compression of neural structures, endocrine disorders, and neuroinflammation, and summarizes comprehensive management strategies based on etiological treatment combined with multidisciplinary collaboration, in order to provide a reference for clinical identification and intervention.
