手术室护士参与器官移植手术心理体验的质性研究

A qualitative research on psychological experience of operating room nurses participating in organ transplant surgery

    目的 探讨手术室护士在参与器官移植手术中的心理体验和真实感受,为医疗机构制定针对性的心理支持方案提供依据。
    方法 应用现象学研究方法,于2026年1月至3月对上海某三级甲等医院17名参与过器官移植手术的手术室护士进行半结构式、面对面深度访谈,录音并观察研究对象,运用Colaizzi七步分析法对访谈资料进行分析、归纳和主题提炼。
    结果 手术室护士参与器官移植手术的心理体验可归纳为5个主题、14个子主题:情感冲击与共情负荷(直面生死的震撼与哀伤、与患者及家属的深度共情、对器官捐献者的敬畏与感动);压力体验与身心耗竭(时效压力下的紧迫感、操作精准度要求的负荷、身心资源的过度消耗);认知评价与意义建构(对职业价值的重新认知、对生命意义的深层思考、对自身角色的定位与认同);应对策略与调适过程(主动学习与经验反思、自我情绪调节与社会支持);成长体验与心理蜕变(心理韧性的增强、专业自信的建立、职业使命感的深化)。
    结论 手术室护士在参与器官移植手术过程中经历着复杂而深刻的心理体验,既承受着强烈的情感冲击与身心压力,也在实践中实现了心理成长与职业认同的深化。护理管理者应关注这一群体的心理健康,构建多层次支持体系,促进其身心健康发展。

     

    Abstract:
    Objective To explore the psychological experiences of operating room nurses participating in organ transplant surgeries, providing a basis for medical institutions to develop targeted psychological support programs.
    Methods Using a phenomenological research method, semi-structured, in-depth, face-to-face interviews were conducted from January to March 2026 with 17 operating room nurses who had participated in organ transplant surgeries at a tertiary hospital in Shanghai. Interviews were recorded, and observations were made. The interview data were analyzed, summarized, and themes were refined using Colaizzi’s seven-step analysis method.
    Results The psychological experiences of operating room nurses participating in organ transplant surgeries were identified as 5 main themes and 14 sub-themes: emotional impact and empathy load (shock and grief confronting life and death, deep empathy for patients and families, awe and gratitude towards organ donors); pressure experience and physical-mental exhaustion (urgency under time pressure, load of operational precision requirements, excessive consumption of physical and mental resources); cognitive appraisal and meaning construction (reappraisal of professional value, deep reflection on the meaning of life, positioning and identification of one’s role); coping strategies and adjustment process (active learning and experiential reflection, self-emotional regulation and social support); growth experience and psychological transformation (enhancement of psychological resilience, establishment of professional confidence, deepening of professional mission).
    Conclusions Operating room nurses undergo complex and profound psychological experiences while participating in organ transplant surgeries, enduring strong emotional impact and physical-mental stress while also achieving psychological growth and deepening professional identity through practice. Nursing managers should pay attention to the mental health of this group and construct a multi-level support system to promote their physical and psychological well-being.
