烟雾病血液生物标志物研究进展

Recent advances in blood-based biomarkers for moyamoya disease

  • 摘要: 烟雾病(moyamoya disease, MMD)是一类以颈内动脉远端及其主要分支出现进行性狭窄、闭塞和继发性颅底异常血管网形成为特征的脑血管疾病。既往研究主要通过检测外周血分子来反映MMD的血管病理和炎症状态,但整体特异性不足、结果受混杂因素影响大,临床应用价值有限。近年来,随着高通量组学的发展,多种具有较高特异性的血液标志物被提出;生物信息学分析与机器学习的发展进一步促进了标志物筛选和诊断模型构建。同时,面向不同临床场景的血液标志物逐渐受到关注,推动MMD血液标志物从“诊断性”向“决策性”和“预测性”转变。本文对近年来MMD相关血液标志物研究进行总结,希望对未来进一步的深入研究和临床转化提供参考。

     

    Abstract: Moyamoya disease (MMD) is characterized by progressive stenosis or occlusion of the distal internal carotid artery and its major branches, with secondary formation of abnormal collateral vessels at the skull base. Previous studies evaluated peripheral blood molecules to reflect vascular pathology and inflammation of MMD, but their limited specificity and susceptibility to confounding factors restricted clinical utility. With advances in high-throughput omics, blood-based biomarkers with improved specificity have been identified, and bioinformatics and machine learning have facilitated biomarker screening and diagnostic model development. Meanwhile, biomarkers for different clinical scenarios have emerged, promoting a shift from “diagnostic” to “decision-making” and “predictive” biomarkers. This review summarizes recent progress in blood-based biomarkers for MMD for future research and clinical application.
