胫骨横向骨搬移技术对糖尿病足的治疗作用及其生物学机制研究进展

Research progress in the therapeutic effect and biological mechanisms of tibial transverse transport for diabetic foot

  • 摘要: 近年来,人口结构老龄化导致糖尿病患者数增加,越来越多的学者将胫骨横向骨搬移(tibial transverse transport,TTT)技术作为治疗糖尿病足的一种选择。TTT是一种以“张力-应力”法则为原理,将力学刺激转变为生物信号,通过改善微循环以促进糖尿病足溃疡伤口愈合的手术技术。TTT技术通过许多生物学机制来改善足底微循环,如通过开窗效应降低骨髓内压力;促进血管生成相关细胞因子分泌,诱导新血管形成;重建巨噬细胞极化平衡,使伤口处的巨噬细胞从以促炎为主的M1型转变为以抗炎为主的M2型,刺激血管生成;激活相关细胞因子通路,促进骨髓间充质干细胞向创面迁移等。掌握机制才能更好地运用TTT技术,因此本文将简要介绍TTT技术的原理与步骤,并对其改善糖尿病足微循环的生物学机制进行综述。

     

    Abstract: In recent years, the aging population structure has led to an increase in the number of diabetic patients, and more and more scholars are using the tibial transverse transport technique as an option to treat diabetic foot. The TTT technique is a surgical technique based on the principle of "tension-stress" effect, which transforms mechanical stimuli into biological signals to promote wound healing in diabetic foot ulcers by improving microcirculation. The technique improves the microcirculation through a number of biological mechanisms, such as skylight opening effect; promoting the secretion of cytokines; macrophage polarization balance reconstruction; and stem cell mobilization. Therefore, this paper will briefly review the principle and steps of the TTT technique, and its biological mechanism of improving foot microcirculation in diabetes.
