PCIA背景剂量对患者术后24 h内舒芬太尼镇痛总需求量的影响

Effect of different background dosages in patient-controlled intravenous analgesia on total demand of sufentanil within 24 hours after surgery

    目的 探讨不同背景剂量下患者术后24 h内舒芬太尼镇痛的总需求量、镇痛效果及不良反应。
    方法 选取2018年1月至2019年12月复旦大学附属中山医院收治的25~80岁接受单纯全身麻醉下手术患者210例,患者术后均使用舒芬太尼进行静脉自控镇痛(patient-controlled intravenous analgesia,PCIA)。根据镇痛泵背景剂量,分为低背景剂量(0.5 μg/h)组(LB组,n=108)和高背景剂量(2 μg/h)组(HB组,n=102),比较2组患者术后24 h内的患者自控镇痛(patient controlled analgesia,PCA)总次数、有效PCA次数、镇痛药物使用总量、数字分级法(NRS)评分及镇痛相关不良反应。
    结果 HB组术后24 h内的PCA总次数、有效PCA次数、镇痛药物使用总量及动态和静态NRS评分均低于LB组(P < 0.05)。2组患者不良反应差异无统计学意义。
    结论 在单纯全身麻醉下手术后行舒芬太尼PCIA的患者中,术后24 h内镇痛泵设置较高背景剂量(2 μg/h)时的镇痛效果优于低背景剂量(2 μg/h),舒芬太尼使用总量减少,且未增加相关不良反应。

     

    Abstract:
    Objective To explore the total demand, efficacy and adverse reactions of sufentanil with different background dosages within the first 24 h after surgery by single general anesthesia.
    Methods A total of 222 patients (aged 25 to 80 years) underwent surgery after general anesthesia were involved in Zhongshan Hospital, Fudan University from January 2018 to December 2019. All patients received sufentanil patient-controlled intravenous analgesia (PCIA) after general anesthesia. According to the different background dosage of analgesia pump, the patients were divided into two groups: low background dosage group (0.5 μg/h; LB Group, n=108) and high background dosage group (2 μg/h; HB group, n=102). The frequency of patient controlled analgesia (PCA) bolus, the number of effective bolus, the total dose of analgesic drugs used, the score of numerical rating scale (NRS), and the adverse reactions related to analgesia were compared between the two groups.
    Results The frequency of PCA bolus, the number of effective bolus, the total dose of analgesic drugs, and NRS score in Hb group were significantly lower than those in the LB group (P < 0.05). There was no statistical significance in adverse reactions between the two groups.
    Conclusion Among the patients received sufentanil PCIA after general anesthesia, the analgesic effect of higher background dosage (2 μg/h) group are better than that of background dosage (0.5 μg/h) group within the first 24 h after surgery, without additional adverse reactions.
