膈肌是人体呼吸运动中最重要的呼吸肌。膈肌功能障碍与患者的肺部并发症发生和预后不良有关,增加患者的住院时间甚至提高死亡率。膈肌超声可通过观察膈肌运动力学,监测膈肌的厚度及其变化率、移动度等来评估膈肌结构和功能,辅助临床医师作出相应决策。膈肌超声在识别膈肌功能障碍,预测机械通气患者撤机、拔管时机,预测术后肺部并发症,评估人机同步性,评估康复效果等方面有潜在价值。 |

Abstract: |
The diaphragm is the most important respiratory muscle. Diaphragmatic dysfunction is associated with the occurrence of pulmonary complications and poor prognosis of patients, which can increase the patients' hospital stay and even mortality. Diaphragmatic ultrasound can evaluate the structure and function of the diaphragm by observing the movement mechanics of the diaphragm, measuring diaphragm thickness, diaphragm thickening fraction and diaphragm excursion, and eventually assist clinicians in decision-making. It has potential value in identifying diaphragmatic dysfunction, predicting the time of withdrawal and extubation of patients with mechanical ventilation, predicting postoperative pulmonary complications, evaluating patient-ventilator synchrony and rehabilitation effect. |
