常规超声联合声触诊组织量化成像技术对乳腺病变的鉴别诊断价值

摘要目的: 探讨常规超声联合声触诊组织量化成像技术(virtual touch tissue imaging quantification,VTIQ)对乳腺良恶性病变的鉴别诊断价值。方法: 采用常规超声和VTIQ技术对2019年4月至2019年9月复旦大学附属中山医院厦门医院62例乳腺病灶进行检查,常规超声观察病灶回声、形态、有无点状钙化、穿支血流等特征,并用VTIQ技术测量病灶横向剪切波速度(shear wave velocity,SWV)的最大值SWVmax及平均值SWVmean,以病理结果为金标准,ROC曲线计算SWVmax及SWVmean的截断值,比较常规超声及常规超声结合VITQ的诊断效能。结果: 62例病灶中,41例为良性肿瘤,21例为恶性肿瘤。VTIQ的SWVmax及SWVmean截断点分别为5.4 m/s和4.2 m/s,SWVmax和SWVmean诊断敏感度、特异度分别为72.0%、92.1%和86.0%、79.0%。常规超声和常规超声结合VTIQ诊断敏感度、特异度、Youden指数、阳性预测值、阴性预测值分别为68%、74%、42%、72%、83%和77%、97%、74%、86%、92%,差异具有统计学意义(P < 0.05)。结论: 常规超声联合VTIQ有助于提高乳腺良恶性病变的诊断能力,值得进一步推广。

Clinical value of conventional ultrasound combined with virtual touch tissue imaging quantification (VTIQ) for breast lesions

Abstract: Objective: To explore the value of conventional ultrasound combined with virtual touch tissue imaging quantification (VTIQ) in differential diagnosis of benign and malignant breast lesions. Methods: Sixty-two breast lesions from Xiamen Branch, Zhongshan Hospital, Fudan University in April 2019 to September 2019 were examined by conventional ultrasound and VTIQ technique. The echo, morphology, punctate calcification and perforator blood flow were observed by conventional ultrasound. The SWVmax and the SWVmean of transverse shear wave velocity (SWV) were measured by VTIQ technique. The cut-off values of SWVmax and SWVmean were calculated by ROC curve based on pathological results. Diagnostic efficacy of conventional ultrasound combined with VITQ was compared with conventional ultrasound for breast lesions diagnosis. Results: Among 62 cases, 41 cases were benign tumors and 21 cases were malignant tumors. The cut-off points of SWVmax and SWVmean of VTIQ were 5.4 m/s and 4.2 m/s, respectively. The sensitivity and specificity of SWVmax and SWVmean were 72.0%, 92.1% and 86.0%, 79.0%. The sensitivity, specificity, accuracy, positive and negative predictive values of conventional ultrasound and conventional ultrasound combined with VTIQ were 68%, 74%, 42%, 72%, 83% vs 77%, 97%, 74%, 86%, 92% with significant difference(P < 0.05). Conclusions: Conventional ultrasound combined with VTIQ is helpful to improve the diagnostic ability of conventional ultrasound for breast lesions, which is worthy of further promotion.

超声检查以其无辐射、方便、可重复性高等优点被广泛应用于乳腺病变的筛查。然而,乳腺良恶性肿瘤的超声表现具有交叉性和多样性,在临床应用中,乳腺病变良恶性的鉴别诊断较为困难,因此超声新技术在乳腺疾病中的应用值得期待[1],弹性成像技术便是近年来应用较广泛的一项新技术。早期的弹性成像是应变式弹性成像,能够获取组织结构的弹性硬度图,从而对组织硬度进行初步判断,缺点是无法进行定量。声触诊组织量化成像技术(virtual touch tissue imaging quantification, VTIQ)是近年来兴起的新诊断方法,它通过反射脉冲激励波对组织施压,计算局部形变后的剪切波速度(shear wave velocity, SWV),从而获取组织弹性的定量特征,最终得出组织弹性量化值,反映组织硬度,为鉴别乳腺良恶性病变提供了更多信息。因此,本研究拟对常规超声联合VTIQ技术在乳腺良恶性病变诊断中的价值进行探讨。

1 资料与方法 1.1 一般资料

选取2019年4月至2019年9月本院因乳腺肿块入院手术的患者,所有入院乳腺病例均行常规超声及VTIQ检查,最终均经病理证实。共纳入62例患者62个病灶,41例为良性肿瘤;21例为恶性肿瘤;年龄23~79岁,平均年龄51岁。排除乳腺囊性肿块、囊实性肿块、粗大钙化灶及肿块位置距离皮下小于5 mm处病例。本研究经本院伦理委员会批准,所有患者均签署知情同意书。

1.2 仪器与方法

使用SIMENS OXANA3彩色多普勒超声诊断仪,常规超声检查采用高频线阵探头(12L6探头),频率范围5~14 MHz,VTIQ检查采用9L4探头。检查方法:患者取仰卧位,双手置于头后,完全暴露乳房,按照外上、内上、内下、外下象限顺序检查,发现病灶后,记录病变回声、形态、边界,并用彩色多普勒血流成像(CDF)技术Ⅰ检测病灶血流。随后切换探头,调节机器模式为弹性VTIQ模式,使超声探头垂直于感兴趣区域的最大切面,检测病变的SWV值。每个病灶检测5次,记录病变的SWVmax和SWVmean

1.3 统计学处理

采用SPSS 22.0进行数据处理,计数资料以x±s表示,采用t检验。绘制VTIQ的SWVmax和SWVmean ROC曲线,计算两者截断值。采用ROC曲线计算VTIQ的SWVmax和SWVmean诊断乳腺病变的敏感度、特异度及Youden指数。对比常规超声和常规超声结合VTIQ诊断乳腺良恶性病变的诊断效能。

2 结果 2.1 临床资料分析

62例乳腺病灶均行手术切除,术后病理显示:恶性病变21例,包括浸润性癌17例,导管原位癌4例;良性病例41例,包括纤维腺瘤22例,腺病伴纤维腺瘤14例、硬化性腺病1例、炎症改变1例及导管内乳头状瘤3例。

2.2 典型病例的超声影像资料对比

结果(图 1图 2)显示:乳腺浸润性癌患者常规超声示肿块边界不清,形态不规则,SWV明显高于周围组织;而纤维腺瘤边界清晰,形态规则,SWV值与周围组织相仿。

2.3 VTIQ对乳腺结节的诊断价值

ROC曲线(图 3)显示,乳腺病灶SWVmax和SWVmean截断值分别为5.4 m/s和4.2 m/s,SWVmean诊断敏感度和特异度分别为86.0%和79.0%。SWVmax诊断敏感度和特异度分别为72.0%和92.1%。

2.4 常规超声结合VTIQ技术对乳腺结节良恶性病变的诊断能力

常规超声和常规超声结合VTIQ诊断敏感度、特异度分别为68%、74%和77%、97%,两者准确性为42%和74%,两者阳性预测值为72%和86%,两者阴性预测值为83%和92%。

3 讨论

乳腺疾病是女性最常见的三大疾病之一[2],临床上常见的类型有乳腺炎、乳腺增生、乳腺纤维瘤、乳腺囊肿、乳腺癌。据文献[3]报道,全球每年新发乳腺癌多达100万,乳腺癌已成为妇女发病率最高的恶性肿瘤。超声具有无放射性、安全、方便、快速等优势,常作为乳腺疾病的首选检查方法,对发现乳腺结节并随访大小具有重要意义。常规超声通过观察乳腺肿块大小、边界、形态、后方回声、周边及内部血流、与周围组织关系及内部是否有钙化灶等声像图像表现来评估肿块良恶性。

乳腺影像报告和数据系统(BI-RADS)分类标准以病灶内部回声、形态、方位、边缘、高回声晕、有无微钙化、穿支血管等形态学指标进行肿瘤分类。刘利民等[4]证实表现多样且不典型的乳腺癌常规超声检查易出现误诊及漏诊。本研究中,经病理证实的乳腺良性肿瘤有22%(9/41)形态不规则,边界不清,内见点状强回声,BI-RADS 5级。经病理证实的乳腺恶性肿瘤有14%(3/21)边界清,形态规则,未测及血流信号,BI-RADS 3~4a级。常规超声出现误诊的主要原因是乳腺占位病变行超声检查时在声像图上与正常乳腺组织有显著差异,在良恶性定性上存在很多重叠性[5]。因此,虽然常规超声是诊断乳腺肿块首选检查方法,但对于一些有交叉特征的乳腺病灶鉴别诊断仍然十分困难。

新兴的VTIQ是一项兼有定性和定量评价功能的技术,由自动检测脉冲产生与声束方向垂直的剪切波,计算出SWV值,并做出彩色图像编码,得出彩色SWV分布图,同时可切换质量、时间、组织位移图,明确直观地显示组织的软硬度[6-7]。不同于常规超声以回声、形态、内部点状钙化及内部血流诊断恶性肿瘤,定性的SWV特征具有高度可重复性,在可疑乳腺肿块中显示出良好的诊断性能[8-9]。VTIQ具有自身的优越性:静态成像,更少依赖操作者,也更客观;感兴趣区更小,仅为1 mm×1 mm,且可多点重复测量;全自动成像,重复性高[10]。以往研究[11-12]表明,常规超声结合VTIQ可提高乳腺肿块诊断效能,因为其提高了乳腺良恶性肿块的鉴别诊断率。其中Tozaki等[11]采用常规超声结合VTIQ对乳腺肿块测得的SWV平均截断值为4.14 m/s,其敏感度和特异度分为88%和93%,与本研究结果相似,本研究特异度较该研究高,敏感度较该研究低。这与研究时的病例组成相关,乳腺病变的特点与肿块直径、深度、后部声学特征、浸润状态和组织学分级有关,因此对VTIQ结果的准确性有一定影响[13]。本研究中,病种以浸润性乳腺癌和纤维腺瘤为主,原位癌和其余类型良性病变例数较少。因此,病例组成会对截断值造成影响,需要多中心大样本量的研究,如能进一步分析出各类病理类型的SWV值范围,将更具深远意义。

VTIQ技术也具有一定的局限性。由于部分乳腺肿块存在不同组织成分的交叉现象,不同组织具有不同的弹性系数,因而易引起误诊[14]。良性肿瘤的病理成分中纤维间质与腺体排列疏松,瘤体内含有丰富的黏多糖,其质地相对较软、弹性系数较低,故恶性肿瘤的SWV值高于良性肿瘤。本研究中有3例SWVmean低于4.2 m/s,提示良性病变的可能性大,但病理结果为乳腺癌,有8例SWVmean高于4.2 m/s,提示恶性病变的可能性大,但病理结果分别为纤维腺瘤、硬化性腺病等良性病变。分析误诊原因:3例恶性肿瘤对照病理显示纤维组织增生较少,瘤体较小,其中1例内部出现液化坏死等使肿瘤硬度下降的改变,故SWV减低;8例良性肿瘤对照病理出现肿瘤纤维化、钙化、乳汁沉积成块等使肿块硬度增加的改变,故SWV增高。均质结构组织SWV测值较不均质结构测值更准确,在不均质结构中SWV测量的有效性和可靠性均下降[15]。可见乳腺超声诊断图像具有复杂性及多样性。因此VTIQ不推荐单独作为常规诊断,但可协助常规超声提高诊断率。

综上所述,VTIQ可提供乳腺肿块的硬度信息,具有较高的可重复性,VTIQ与常规超声相结合,有助于提高乳腺病灶的诊断准确性,值得临床推广。
