视辐射解剖研究进展及其在神经外科手术中的意义

摘要:视辐射是视觉传导通路的重要部分,将视觉信息从外侧膝状体传递至枕叶视皮质。在神经外科疾病和手术并发症中,因视辐射损伤造成的视野缺损并不少见。通过一般影像解剖研究、纤维束剥离解剖研究、弥散张量纤维束成像研究,人们对视辐射解剖的认识逐渐深入。这些解剖学知识被应用到相关的手术设计和功能保护手段当中,加强了术中对视辐射的保护。

Advances in anatomical study of optic radiation and their significance in neurosurgical procedures

Abstract: As a substantial part of the visual pathway, optic radiation conveys visual signal from lateral geniculate body to the occipital cortex. It is not rare to see reports of optic radiation injuries as consequences of neurosurgical diseases or neurosurgical complications. Trough general anatomical or radiological study, fiber dissection anatomy research, diffusion tensor fiber tract imaging research, the anatomical knowledge has been deepened and expanded. With all the new information added to the neurosurgical procedures, the intra-operative protection of optic radiation has been strengthened.

视辐射是视觉传导通路的重要部分,将视觉信息从外侧膝状体传递至枕叶视皮质。视辐射在大脑内途经颞叶、顶叶和枕叶,涉及范围广,在很多神经外科手术中都有涉及。然而,目前很多外科医生对这一解剖结构认识不足,在术中缺乏对其保护的意识与方法。

1 解剖行径

视辐射是视觉传导通路的重要部分,自外侧膝状体发出,止于枕叶视觉皮质[1],可分为前束、中束和后束三部分[1-3]。前束自外侧膝状体,在侧脑室颞角顶向前、外侧延伸,然后向后急转形成Meyer袢(Meyer’s loop),走行于侧脑室颞角、房部和枕角外侧壁,并跨过房部和枕角的底部向内侧,到达距状沟以下的视皮质,负责传递对侧上方1/4视野的视觉信息[4];中束向外侧跨过侧脑室颞角顶,向后走行于侧脑室颞角、房部和枕角的外侧壁,到达枕极,传递黄斑区的视觉信息,在三束中体积最大[5];后束自外侧膝状体直接向后走行,经过侧脑室三角部的外上方直接到达距状沟以上的视皮质,传递对侧下方1/4视野的视觉信息[2]

2 视辐射损伤的常见原因

视辐射损伤造成的视野缺损,常出现在神经外科疾病和手术并发症中[6-7]。所有涉及视辐射行径的创伤性颅脑损伤,包括顶枕部硬膜外血肿、颞顶枕部挫裂伤、弥漫性轴索损伤等,会造成不同程度的同向偏盲[8-9],新的影像学方法也用于揭示这一过程中视辐射的变化[10-11]。有研究[12]报道颞叶动静脉畸形的手术病例,记录了相关出血造成的视野缺损,以及这些视野缺损的恢复情况。侧脑室房部占位亦可累及视辐射、造成视野缺损,经过颞叶上部、顶叶下部进行手术切除,也可以并发不同程度的视野缺损[2]。作为手术并发症,视辐射损伤最常见于癫痫前颞叶切除术后[13-14]

3 视辐射的解剖研究

视辐射的解剖研究,主要通过两大类研究方法完成。一类是针对尸体标本的解剖学研究,另一类是针对活体受试者的影像学研究。其中,纤维束剥离技术和弥散张量成像(diffusion tensor imaging,DTI)技术对视辐射解剖研究最为适用。

3.1 一般影像解剖研究

在对颞叶手术切除范围和术后对视野缺损情况的研究中[15],神经外科医生对视辐射的解剖有了最初的认识。Barton等[16]将术后MRI用于切除范围的测量发现,视野缺损的程度与颞叶切除的范围显著相关。Burgel等[17]的视辐射解剖研究,则运用了白质纤维染色、断层解剖、MRI、3D图像重建等技术。Kier等[18]发现Meyer袢前方并没有到达颞角的尖端,这与Barton等[16]的结论相矛盾。Schur的团队[19]则基于特殊的MRI T1扫描数据,对视辐射的纤维束进行了满意的重建。

3.2 纤维束剥离解剖研究

纤维束剥离技术在解剖学中的应用,可以追溯到17世纪。这一技术越来越多地被应用于视辐射的解剖研究中[20]

3.2.1 解剖研究进展

有研究[4]分析视辐射与侧脑室颞角、颞叶表面结构的相对位置关系发现,侧脑室颞角的外侧壁、顶部、尖部的外侧部分均为视辐射所覆盖,特定区域的颞角底则没有视辐射纤维。Rubino等[3]发现,视辐射纤维与侧脑室颞角顶之间仅仅有一层毯纤维隔开。Mahaney等[2]则发现,侧脑室房部的内侧壁没有视辐射纤维覆盖。Parraga等[21]也研究视辐射与侧脑室的关系发现,前后方向上,Meyer袢的最前端始终在颞角尖端以前;上下方向上,视辐射在颞上回、颞中回的深部,向下不超过颞下沟。Atar等[22]则将3.0T MRI与纤维束剥离技术结合起来,显示向枕叶走行的视辐射的解剖。

3.2.2 优势与不足

纤维束剥离研究极大地丰富了人们对视辐射解剖的认识,但这项技术也存在一定的局限:(1)只能在尸体标本中进行;(2)无法将交叉、伴行的纤维束与视辐射分离;(3)在剥离出一组纤维束往往意味着破坏了邻近结构;(4)标本处理过程中的冰冻和融解会造成一定程度的标本变形[23-28]

3.3 弥散张量纤维束成像研究 3.3.1 成像原理

常见的MRI成像技术呈现出的白质,往往是是无法分辨、同质性的白质组织团,难以使某个具体的纤维束显像。弥散MRI的原理,基于轴突中不同方向上,水分子弥散运动的活跃程度不同。DTI是弥散MRI中的一种,以DTI数据和现有的解剖学知识为基础进行纤维束成像,从而将混为一体的白质结构显示成行径清晰的纤维束[29-30]

3.3.2 解剖研究进展

Nilsson等[31]对前颞叶切除患者和正常人进行了Meyer袢的纤维束成像研究发现,Meyer袢前缘位于颞极后方34~51 mm(平均44 mm),向前未到达侧脑室颞角最前方,并具有相当程度的个体差异。研究人员探究了钩束、前联合、枕额下束、丘脑下脚和视辐射的空间关系,定义了“岛叶后点”(Heschl颞横回与下环岛沟交界线的最前端)这一解剖标志,将其作为视辐射的后界。Wu的团队[32]将重建的视辐射纤维与T1图像重建的立体脑模型进行融合,发现视辐射走行于颞上回、颞中回和颞上沟深面,与下纵束、顶/枕/颞桥束关系密切。

3.3.3 成像与重建方法进展

Kuhnt等[23]发现,应用高角分辨率弥散成像(high angular resolution diffusion imaging)数据对胶质瘤患者的视辐射进行重建的效果,优于传统DTI数据,但需要更长的扫描时间。Lim等[33]研究则发现,采用约束球面反褶积(constrained spherical deconvolution,CSD)技术进行纤维束成像,在缩短运算时间的同时,还可以获得更好的重建效果。Chamberland的团队[34]通过引入一种新的感兴趣区域(region of interest,ROI)机制,使更多的Meyer袢纤维得以显示,缩短了所能重建出的Meyer袢最前端与颞极的距离。

3.3.4 优势与不足

纤维束成像的优势在于能够在活体、针对患者的视辐射解剖进行无创的、个体化的评价[25],计算机技术的发展让此技术变得更加自动化[23]。但是,纤维束成像存在以下一些问题:(1)弥散磁共振扫描时间一般偏长,在此过程中,患者移动、磁涡流、信号噪音等因素都可能给数据的准确性带来影响[25]。(2)数据处理的方法可能存在注册误差和算法局限,有时重建效果与实际解剖不符[32]。(3)纤维束成像的“分辨率”存在局限,其体素(最小成像单位)的大小,比构成神经束的轴突直径,存在数量级的差距[16]。从某种意义上讲,此方法呈现出的是“大体结构”,而不是“显微结构”,与实际解剖的效果尚无法比较[22]。对于相互混合、交叉、吻合的纤维束,造成同一体素内走行多个方向的纤维束[4],或者对于比体素还要细的纤维束[35],纤维束成像难以进行满意的处理。(4)纤维束重建选择ROI时,依赖重建者的主观判断,从而使重建存在一定的主观性。

4 视辐射解剖研究的手术意义

对于神经外科医生来说,保护视辐射最根本的方法,就是根据对自身解剖与毗邻关系的理解,选择甚至设计最佳的手术策略。

4.1 占位性疾病手术中的视辐射保护

侧脑室房部与视辐射的密切关系,一系列手术入路,包括经顶上小叶入路、远端外侧裂入路、经颞下沟以下的皮质入路、经纵裂入路等,被开发出来避免相关手术中的视辐射损伤[28, 36-37]。Faust等[38]总结了经手术治疗的130例颞叶肿瘤病例,结合术前DTI成像和术后视野缺损情况,分别给出切除颞叶外侧、颞极、颞叶内侧、颞叶中央和梭状回等部位病变,为了保护视辐射应选择的最佳入路。

4.2 癫痫前颞叶切除术中的视辐射保护 4.2.1 安全切除范围

视辐射前束,即Meyer袢,在颞叶所能达到的范围,在视辐射解剖研究中最受关注。因为这直接关系到颞叶手术中可以“安全切除”的范围。现在一般认为,Meyer袢最前端与颞极的距离在30~40 mm,其位置存在一定的个体差异性[24]。最常用的2种研究方法都存在其局限性。纤维束剥离研究:因为难以分辨视辐射与邻近纤维束,得出的视辐射范围往往偏大、位置往往偏前。纤维束成像研究:因无法保证所有Meyer袢纤维的成像效果,得出的范围往往偏小、位置往往偏后[29]。因此,目前关于视辐射的范围与最前端的位置、Meyer袢最前端与颞极和侧脑室下角的位置关系,都还存在争议[1, 24]

4.2.2 手术入路选择

前颞叶切除可以选择的手术入路,包括经外侧裂入路、经外侧皮质入路和颞下入路[3]。(1)经外侧裂入路由Yasargil[1]最先应用,可很好地避免视辐射损伤引起的视野缺损,同时可以避免颞叶皮质及引流静脉损伤[4]。Choi等[39]通过解剖研究,提出一个由外侧裂进入侧脑室下角的安全三角区。然而,Yeni等[40]则报道了与之相矛盾的手术结果,同样的手术入路造成了高达36.6%的视野缺损。(2)在外侧经皮质入路中,为了避开视辐射纤维,可选择切开颞下沟以下的颞叶皮质[3]。(3)经颞下入路、由颞叶底面切开进入,可能同样可以避免视辐射的损伤,因为有学者认为侧脑室下角的底面没有视辐射纤维的通过[3]

5 预防视辐射损伤的技术方法

可预测、预防视辐射损伤的技术方法包括视觉诱发电位、神经导航、DTI纤维束成像等[41-43]

5.1 影像学方法

Yogarajah等[29]通过对健康对照者和前颞叶切除病例进行纤维束成像研究,发现通过测量切除范围和视辐射前缘的位置,可以很大程度上预测野缺损的发生和严重程度。Winston等[25, 33]比较了视辐射纤维术成像的不同方法,并将纤维束成像的结果与术后T1序列的MRI影像融合比较,发现视辐射最前端和颞叶切缘之间的距离与视野缺损程度显著相关。同一团队在后续研究中同时应用了术中核磁共振[44],并报道了一组通过术前的纤维束成像评估手术收益和风险、把握手术指征、选择手术入路的病例[45]

5.2 多模态神经导航

Kamada等[33, 35]将视觉诱发电位和DTI纤维成像导航结合起来,监测后视路旁病变切除术中的视路功能,可较好地预测术后视野缺损,并一定程度上在术中为外科医生提供重要的参考。Chen等[15]进行了术前和术中的磁共振扫描和纤维束成像,除对术后视野缺损的情况与纤维束成像结果的相关性进行研究,还着重对导航的术中漂移进行统计。Thudium等[25]报道一组通过颞下入路和经颞叶皮质入路手术治疗的颞叶癫痫病例,将包括Meyer袢在内的重要结构重建后直接显示在显微镜视野中,为外科医生提供“直视下的导航”。近年来,又有一系列研究[22, 34, 42, 46-49]表明,术前或术中的DTI纤维重建技术,能够满意地预测由视辐射损伤引起的术后视野缺损,并能有效预防视辐射损伤的发生。

综上所述,长期以来,神经外科对视辐射这一结构的认识和保护都相对欠缺。一方面,其损伤带来的视野缺损,尤其是轻度的视野缺损并不易被发现。另一方面,白质结构相互交叉、伴行,难以辨认,加之可能存在的病理性移位和个体差异,使外科医生在术中难以判定视辐射的位置和行径。视辐射的解剖研究,尤其是纤维束剥离和弥散张量纤维束成像的进一步发展,让人们对视辐射这一结构的认识大大加深。
